I have a guide book and some plans of what to do but thought if others had been they could give me some 'must do' things

Vatican museum is excellent, but don't run through it just to take a load of pics of the Sistene chapel. There's loads of amazing stuff in there if you know a bit of history, and the private guided tours are even better. The Forum is quite cool too.
However if someone stops you and starts giving you some smooth talk, tell them to fk off. Con artists on every bloody corner 🙁
Look at Italian women. Eat Italian food.
The forum and the hill bit ( forgotten what its called) with all the ancient ruins amused us for two day along with the Colosseum. Open top guided bus was worth doing as well

I was there for 4 days about 3 weeks ago, awesome place.
Vatican museum, St Peters, Colesseum, Forum and the Pantheon are must dos. You can also take a lift up to the top of the huge white building (can't remember name - was monument to the first king)
Lots of lovely restaurants, the ice cream is immense too.

Tell that to all the ignorant tick-list tourists barging through the map halls at a run with their digicam held out... It was full of people taking photos when I went.
Villa Borghese and the Borghese gardens is great for a half a day or so. You have to book in advance to get in the Villa, it is an immense private art collection amassed by the scheming Borghese family, and now displayed in their former residence and open to the public. It's very good. The park it is set in is great too. Sculptures round every corner.
We hired a two person pedal buggy and hooned around in the park for a couple of hours finding the sculptures, it was great!
I'd give the spanish steps a swerve if you are pushed for time. We were unimpressed. The area they are in is lovely for a wander round the shops and a leisurely lunch in one of the restaurants, pricey round there though.
The stuff listed above is all great too. Also take some time to just wander around, it is an amazing city to just spend time in, so much history just right there around every corner, beautiful courtyards and cafes everywhere.

Eat ravioli mimosa at taverna trilussa.
Dont be put off by the queue at musei vaticani - goes quick, and totally worth it.
Lots of nice bars, restaurants and lively drinking atmosphere in Trastevere..I'd suggest here if you want a night out
1. Don't get you're photo taken with the guys dressed up as roman soldiers, they won't give you're camera back till you shell out €20
2. Don't be tempted to look in the bag of the guy (he's in a rush) selling the beautiful leather coat. The bit he let's you see quickly is "beautiful leather" the rest is pleather!

Go the Pantheon at night and sink a cold beer whilst sat on the fountain. No idea if you're meant to but we did and it was magical.
The Vatican is stunning, spend a good day there, it is well worth it.
Wander around, Rome is small enough that you can just wander around and stumble upon stuff.
Eat at Dell'Omo, a tiny trattoria 1 block from Termini, is looks like someone's kitchen but the food is stunningly simple "proper" Italian food cooked by Mama and served by father and sun.
The Forum and surrounding area is worth a day mooching around as well.
+1 for wander around. I have been in Rome quite a bit, and love the opportunity just to, well... roam.
The Trevi Fountain at night is lovely.
I'd highly reccomend the open top bus tour as well. It's a really good start to a trip there as it gives you a feel for layout and locations of things and you can jump on and off at will.
A generaly wander around the place is very worthwhile too as there are so many old buildings and remains around the place to see.

If start at the colloseum the entry ticket gets you in (or on) to the palantine hill, home of the caesars (in fact the word palantine is the etymological origin of palace and palatial). Worth doing if it's a nice day for the views and the museum. nice view over the forum as well. down into the forum for some ooh'ing and aah'ing if you're into the history of rome (less so if you're not). There was an interesting exhibition on in the curio earlier this year comparing rome to china with some of the teracotta warriors on display. anyawy, out of the forum, quick looka t trajan's forum on the other side of the road, find a decent vantage point and watch the chaos of the piazza venezia. then off up via corsa. a number of churches ghhere, but specifically the Chiesa di Sant'Ignazio - not on via corsa, off to the left as you walk away from the typewriter/piaza venezia. the ceiling is fairly impressive

there's a michaelangelo statue in a church on the right hand side of the via corsa. the pantheon isn't too far away and well worth a visit to ogle the engineering feat as much as anything else. eventtually, you'll get to the top of the via corsa (you can detour to the spanish steps if you like, impressive but full of people selling tat) and there'sa church at the old wall (chiesa di santa maria dei poppolo iirc) which has a couple of caraviaggios - well worth seeing. at night check out the trevi fountain - spectacular and tourist infested, but amazing, especially when it's lit up (colloseum and imperial forum also worth checking out at night).
[url= http://en.wikipedia.org/wiki/Basilica_of_St._John_Lateran ]san giovanni in laterno [/url] is a 1 euro bus trip (buses run from the poppollo end of via corsa)and was the home of the papacy until it moved to the vatican. in the same league as st peter's but without the crowds. you should still visit st peter's though, even if only to see michaelangelo's pieta.

also, across the road is a church which houses the [url= http://en.wikipedia.org/wiki/Scala_Sancta ]santa scala, or holy staircase[/url] or stairway to heaven. But remember if you go for a swatch, you can only climb it on your knees and reciting some mumbo jumbo. fortunately, ther parts of the scala to the feft and right of the central section are less holy, so you can just walk up them. not much to see att he top other than a 4th cenntury piece of icongraphy which was created by an angel. apaprently.
vatican museums are ok if you don't mind queueing for 3 or 4 hours (yes, worth it)

Cross the bridge by the little island in the middle of the river and there are some lovely bars and restaraunts on the other side and not so busy or pricey.
Check our Via Condotti to people watch all the fashionistas going into the designer shops.
