Your suggestions of cover that actually covers and at a reasonable price.
Family of 4, just over two weeks in Canada canoeing on flat water, some hiking and Whistler/Pemberton on bikes (natch).
I've just run a load of quotes:
- initial varied between £200 and £980(!), all rocketed in cost or didn't cover enough
- three of the five included mountain biking but had the following qualifying statements: 'paths not trails', or 'not extreme' or 'marked routes only with no stunts'
- the other two had us confirming it is downhill mountain biking
- three doubled in cost when I added minor high blood pressure for me and a medical issue for mrs_oab (which on non 'extreme sports' cover adds 5%)
So, where to look?
Always used dogtag when going to Canada, never had to claim though and it’s just the two of us. Has included mtb racing as well.

I appreciate that. Some of the expensive ones don't cover so well, and there is a balance of cost as well...
For the last couple of years I have used Columbus Direct with the added adventure cover as an annual family travel insurance policy. The main reason for me (beside price) is that it covers scuba diving to 40m, which most off the shelf policies don't. I haven't read the small print in detail to see what it covers for off road riding, but worth a look if you haven't tried them already.
I'll keep an eye on this, we need to get something sorted for Sri Lanka next month, including a week kitesurfing and likely to be biking again somewhere in Europe. I usually do it at the last minute and just get single trip cover but think an annual policy would be worthwhile.

We used them last year. This year they are looking good but pricey (£320 after cashback).
Yellow Jersey annual for us. Some of Abigales friends have had to use them whilst competing and they have been first class sorting them out.
I always ring them and ask for a discount code. Every little helps

I have the premium Virgin insurance, just over £100 for four of us (including one son who is over 18 which is a problem for some companies) with winter sports cover.
I have a feeling holiday insurance will rocket in the future when we come out of Europe (unless we agree some kind of reciprocal arrangement with EU countries). All of those free hospital visits for holiday makers will probably have to be paid for by the insurance companies.
Edit- Just checked the policy and it states the following are included:
Mountain biking (not including downhill racing and extreme ground conditions)
Canoeing (not sea canoeing)
Hiking (under 2,000m)
Hiking (over 2,000m but under 6,000m altitude) but this excludes personal liability
