Is it possible to be a working Paramedic and not appear on the HCPC register?

If so, under what circumstances would this be the case?

Not legally in the UK no. It's a registered profession and a protected title.

However... Not everyone in green is a Paramedic. Nurses, Doctors, Army combat medics etc often fill roles (particuarly on private vehicles). Its a bit of a murky underworld full of loopholes and clauses. But as Drac says, anyone identifying specifically as a Paramedic should be registered and identifying as a Para without registration is a legal issue. Reg number should start PA00000 (o's being reg number)

quite a few impersonators have been found and fined, real ones usually have an embroidered badge on there jerkins, romper suits. if in doubt ask for id.

There's a scary number of wannabe oddballs about, and some are more convincing than the borderline afflicted individual documented below. A strange obsession, for sure.

Its more of an issue with someone befriending a vulnerable teenager on facebook. Something doesnt smell right about him (a lot of his friends are 17 to 18 yr old girls in a different (particular) part of the UK to him), he is late 20s. He is claiming to be a paramedic, but he doesnt come up in the HCPC register online.

Hcpc will happily do nothing about him, police may be interested? Or,friend him with a fake profile, invite him round, chop his hand off and see if he can manage major traumatic haemorrhage inline with jrcalc 2016.
