I'm in the middle of fitting a new bathroom. The new toilet doesn't quite fit exactly where I want it. I can make it work if i modify the holes a little. I need to move the water outlet on the cistern about 20mm. Here's the crazy plan: put a piece of wood in the existing hole, use that to centre a core drill and cut a new hole to effectively elongate the existing hole. I then need to fill the back of the hole with something more waterproof. So, any chance of the drilling working? Im thinking diamond core drill and water. What to partially fill the hole with? Any other ideas. I have the old cistern to test drill.

Which water outlet?? The flush? If so I’d be suprised if it worked. If it’s the overflow then get a syphon with built in overflow.

I think there's a reasonable chance of it working, although I wouldn't try it unless there's no other way (and I have never done it). I'd fill the existing hole first, using epoxy putty (many types to choose from that can be used with ceramics) and then diamond core the new hole (wet or dry as per the drill instructions). The drill bit will work better in a consistent material, trying to drill half a hole is likely to be a problem.

I’ve drilled and modified in the past, I used a Rubi Dry diamond hole saw on a grinder.  I’d fill the old hole 1st the re drill once dry.
