I have a document I'm writing (about 200 pages).
The first page is the title and the following 3 pages are index.
How can I get the page numbers in the index change automatically when I add or remove pages from chapters?

If you right click on the index table there should be an option to update page numbers.
Sorry for being stupid but how do you get an indexed table?
From the menu line 'Insert | Index and Tables | Table of Contents'
John John is right, but for that to work you will also need to make use of the proper numbering format for headings etc - not just type 1., 2. etc.
