Yep, the photo shows somebody who is in the middle of facial expression that looks angry. That is what you can take from the photo. People have applied meaning to it, unless the person in question has commented about that nobody really knows.
How does it become a reused meme? The sites that let you easily create these store the pics for other people to use, so once it has a bit of a following it will come up more and more often.
