The addition of the second word makes it likely to be derogatory. Therefore insult. Unless some fella was caught knocking one out in the golf club toilets in which case it could just be descriptive.

It's a mixed blessing...

Just when you're reveling in 'middle-class' - you're just glad someone noticed that you studied ethics at university and occasionally listen to jazz - they hit you with ****erish. You're not exactly a ****er, but you're ****er-ish, which surely is worse.
