Any suggestions? Simple to use, not too many words in the interface, preferably with some pretty designs included like maybe simple clip art.
MS Paint?
Tux Paint is designed for children, so should be really easy to use. The website seems to be down just now, but I'm sure you could find a download somewhere. http://www.tuxpaint.org/
What age and ability? iPad/tablet apps and a pen are the best to allow that creativity flow.
If you need something for a pc/mac get hold of a copy of old illustrator, it's suprisingly easy to use and good experience for the future.

Ummm... KidPix is available for Windows OS.
ArtRage
has quite a lot of depth but also has stencils clip art and plenty of other fun stuff.
Tracing over imported images is a winner 🙂
Interface does not take long for young guns to get used to as more icon than menu based.
+1 artrage, although is more painterly than txt-y.
