I keep getting horrendous sharp pains in my thigh and quads at the start of every ride. For the first few miles it's agony. Now I have had a knee arthroscopy (knee debridment) now I know I'll have some pain but it's in both legs. And if I rest for a few days it's even worse the first ride back.
I thought it maybe crap diet or lack of water but I drink plenty of water and maybe a can or two of diet cola now and again. Any ideas.
Do you stretch before riding?

Try going to see a sports masseur. Did wonders for my calves.
Is probably your legs warming up then. When I go for a road ride the first 10 miles or so are usually so painful I consider going home early.

Then there must be something seriously wrong with your set up or are you hammering it right from the start?!

Can you get someone who knows what they're doing to check it?
