I'm pretty sure X12 is the 12x142 - the 12mm conversion is the 12x135 kit. That's what I got and it works :).
Evo hub only needs end caps - the pre-Evo one needed new axle etc. but the Evo comes with an axle that works with 12mm.
I've just called Hope to double check, it is the X12 kit I need. Cheers.

Just to clarify-the OP's situation is the same as mine? I Have a EVO 135mm QR hub and need to make it 12x142 so I need the X12 kit? (to slide the Rockshox maxle through.
You are right. The complications arise if you have a pre-EVO version of the Pro2 - you need a new axle etc. For your evo, you just need the right end caps.
As above X12 is the 142x12 conversion - the 12mm one is the 135x12mm kit (older standard where the wheel doesn't locate in the dropouts before the axle is put in).
