Fiamma do some racks that clip to the doors (tailgate or barn) of various vans. Have a look at their range. I can't think of any other options to be honest. not sure of any one else who makes them certainly.
fiammo sorted me out thanks, was for a friend who frequently transports loads of club bikes around hence internal + external storage needed. thanks steve
I've got a three bike Fiamma rack on my T4 - works really well. Can even open tailgate with a couple of bikes attached. Pretty sure they do a 4 bike version too.
