For years I've ridden XC style mtb shoes with spds on my road bike. My last set of Mavic Rush shoes lasted 8 years and after initial setup I never had to fiddle with the cleats that whole time (replaced like for like position of course).
This year I replaced with some Giro Sector. They seemed fine for the first few months until the left came loose and I had to reset it. Now, months later I've tried many, many different positions and can't avoid pain on my left foot (right foot has been fine this whole time).
The pain is on the outside of my left foot, around the yellow highlight here (about middle from front-to-back)

In the latest position I've slammed the cleat back, and the shoe as outward as possible like this:

Still no help. What to try next? New position? Wedges? Insoles? New shoes?

I'm the same. Dunno if it would help but I've used shimano shoes for years, then tried some Giro shoes. Couldn't get the cleat position right. Eventually had to extend the cleat slots rearward by about 3mm with a dremel so I could position the cleat further back. Now fine for me.
i get an ache in a similar spot on the outside of the foot if my shoes are laced too tight. even if by just a small amount. so could be the shape of the shoe and/or where it puts pressure on the foot when tightened. not obviously tight to start with but after 30 mins or so of riding it can cause discomfort. just a thought.
Try and make sure the ball of your foot is over the axel - I'd set a pair of shoes up slightly too forward and it gave me hot spots. I've got to sort out two new pairs soon.
I always go for as far back as possible. Are you using the same pedals? If the new shoes have a less stiff sole it might be your foot bending round the side of the pedal
Agree w Fossy about ball over axle.
Have used giro insoles in shimano shoes on road and mtb.they have an adjustable insole/arch lift allowing you to tweak foot lateral height and position allowing a more even pressure across the foot
Might be worth giving that a look
Don't think you can do cleat wedges on spd can you?
