Cable tension adjusted when I'm middle ring position and largest sprocket so that the chain brushes the mech inner plate

Mech now shifts well into inner ring and up into middleÂ
Slow to shift to outer position so undid high limit 1/2 turn.Â
Shifts better into outer but not perfect.Â
This is where the fun begins.....
Bit more tension in the cable and shifts to outer well

Now shifts poorly into the middle position and rides in the gap between the 2 rings!

Where an I going wrong? Yes I know it shouldn't be a triple but it's a customers bike not mine!

Is it definitely a triple shifter and you aren't just using the trim positions with a double? I had this with a customers bike, it came with a double shifter and chainset but he changed the chainset to a triple. When he brought it to me for a new chain and cassette I assumed that's how it was supplied and spent ages trying to resolve the same issues you describe before the penny dropped.Â

The cassette seems quite large for a triple chainset? Is this causing the issue?
One or more chainrings fitted back to front?
Every time I've seen the chain caught between two rings this is usually the cause.
That was my initial thought but no, all 3 are correct orientation.
