So does this exist actually and what does it cost (ballpark)?

I mean expert surgery/rehab/physio etc like a footballer or top rider gets or is this part just paid for privately by teams.

e.g. My medical insurance is 6 physio per year and everything needs to be referred by a GP but when some pro's have an accident he/she are straight into surgery and sports rehab whereas others are on go-fund me.

A team doctor will do the referral, and then the insurance premium will depend on the risk of serious injury

Ah.... makes sense.
The bit I'm most interested about though is getting the medical stuff that gets you back riding ASAP with no other considerations.

Like basically if I didn't mind about scars or being able to lift my arm in 5yrs... is there a way to just bypass this and pay ??

You could find the surgeons the pros go to and get them to do the work privately? cost and arm and a leg tho 😉

The first time i did my shoulder i was told that if I were a pro athlete I would have a surgical repair but for an ordinary bod the risk / reward balance is different so not worth it

Yeah as per some other current threads a lot is perspective on risk/reward.
At my age I feel a bit like the time I have left to "send it" is ticking away whilst the consequences of prioritising riding get less.
