Fitting a new set of Middleburn cranks (replacing an old pair). The drive side fitting and removing bolt has sheared off completely....at the bolt head. This has left the full threaded length in the crank. Any advice????

I'd try removing the extractor cap first, and then try undoing the remainder of the bolt, chances are it'll unscrew easily if it sheared when you were tightening it.
This happened to me. Middleburn used an alloy instead of ss bolt for a short while until it became clear that they were far too soft and breaking.
They sent me a new set.
If you able to remove the crank I'd suggest then removing the bb from the frame and taking it to a good bike shop or your local engineer's place.
I had this problem once, with a Ti Royce BB. Local machinist sorted it for a fiver. I dropped off some beers for him later.
can't get the crank off the frame :(. The bolt has sheared at the head and unable to get any purchase on the exposed thread to fit the replacement bolt or remove the left over thread.
Will need to try and find a local machinist, as per Ambrose advice, who will be far more capable than me to use something like yourguitarhero recommendation.

I have used a car BB splitter on an aluminium frame, just hammered it between crank and frame....
A bit brutal but it works
