I'll second Espuna Enduro, Pitu is very knowledgeable and the area is fantastic. No tarmac smooth bike park stuff, but lots of amazing single track.
Saddle Skedaddle. Just did their Tranz Andaluz trip (Spain) and have to say very impressed with it all, and very well organised. They do trips to loads of places.
They do a wide variety of types of trips, both off and on-road. This one was off road, more AM/XC, point-to-point tour and some (very rocky) singletrack. Raw natural singletrack, which I prefer. Might try a singletrack focused one, but a whole week of that stuff could be hard going if it was like the singletrack we were doing.
Ben Jones Mont Blanc Enduro is excellent.
Anyone got recommendations for Himalaya?
