Even on short rides I am getting pain in the underside of my thumbs, I have changed straight bar for riser and tried different grips, any ideas?

Try rotating the bar very slightly down. that will shift the pressure/pinch point slightly in your hand and may ease it.
Only a very slight rotation initially though.
