My summer shoes were fine last year.
This year my right foot gets painful after 4 miles.
Have specialized insoles so they obviously aren't the solution.
Have big spongy insoles and they don't help.
Should I be looking elsewhere for the cause?
Seat height etc?
Are carbon soled shoes going to help?
It took me over 10 years to fix mine. Tried all sorts of shoes, inner soles, saddles, saddle height. Then I moved the cleats back as far as they’d go. Sorted.
Where is the hot spot?
Right at the front of the ball of my foot going into toes.
Yup, same thing for me too. +1 for moving the cleats back.
I like solutions that are free.
I can't guarantee but it should help. 😀 Worked for me. This position also feels a bit more natural IMO.

Metatarsalgia? I'm pretty sure that that's what I have (in both feet) and it's really painful; I'm having real issues sorting out mine
I've spent the last 2 months at work either barefoot or in flip flops.
Wondering if that has altered my feet.
As I said , my shoes were fine last year.
