Set it up using the red plastic guide about a year ago and all has been fine until a cable replacement recently. Now I have a slight tick-tick noise in one gear despite barrel adjustment tweaks. Do I need to get the red thingamejig out again?

Which gear - if it's in the middle then it's likely some wear to the sprocket - it would happen in more gears if the adjustment is out. Also check no dirt has got in the way - good mech clean.
