I get a pain in the outside of my foot, on/under the little toe joint, of my right foot. It's normally not too bad, but first ride with new, stiffer, shoes yesterday (Shimano RX8 shoes/Time ATAC cleats/pedals) and it was really noticeable. (I also remembered yesterday that I get the same pain, but much worse, in ski boots).

A bit of googling this morning suggest that it's pretty common, and often caused by a varus tilt in the foot, which can be alleviated by cleat wedges. Anyone had/used similar to good effect?

Me, I use cleat wedges and internal wedges under my insoles, these.
They were fitted and set up by a proper bike fitter and pretty much fixed the exact problem you mention (still get the odd bit of discomfor at the end of really long rides, but wriggling my feet gets rid of it). You're messing with body mechanics so do be careful but for me, they did work.

As for your ski boots, the only fix I got was spending far to much time with my boots and feet in the hands of Colin at Solutions4Feet

Interesting, cheers.

I had it with my previous shoes a bit, but dropping my saddle about 5mm seemed to fix it (I think I was overstretching a bit at the bottom of the pedal stroke, which caused my foot to roll outwards). Thing is, as soon as I tried the new shoes it started again. I can't see that the difference in sole thickness between the two is that much, but I've dropped the saddle another couple of mm so we'll see.

Failing that, I've seen a set of 1deg cleat wedges on ebay for a fiver so I may get some of them and have a play.

So many variables though; new shoes, stiffer shoes, new cleats (the others were reeeeeally worn so quite sloppy), cleats probably in slightly different position (I've got them bang dead centre of the guides on the bottom of the shoe for now). Saying that, one of the advantages of hours on Zwift is it doesn't half highlight issues with fit and allow you to tinker with it in a fairly controlled environment.

specialized internal wedges work a treat. i found the external ones can sometimes interfere with the pedal mechanism
