There is a cycle death every 3 days or so (122 in 2012: http://www.thetimes.co.uk/tto/public/cyclesafety/article3313260.ece)
It's not an unusual event, statistically, regrettably.
In fact recognising the causes as systemic, rather than a special case each time (obviously it's tragic and special for the people concerned), might help to reduce the number in the future.

I wondered why the road had been blocked. Traumatic for both riders family and driver. After all we don't know for a fact who was at fault.

- further to benjii19, this time it sounds as though driver may not have been culpable, if the following is correct:
... pushbike, being ridden ... in the opposite direction, reportedly emerged from the grass banking to the nearside of the Fiesta.
regardless, it's awful for those involved.
