Previously I have used the Shimano ESHR320IN insoles - they have two height arch inserts and another sticker to put under toes. Always worked for me, mega comfy and give the support just how I like.
They are discontinued so does anyone have cost effective alternatives - I paid about £30 or so for each set before.
I always liked the Specialized BG insoles, don't know if they are available sans shoes. Otherwise, Superfeet are good for general support and have different types
I see some SQLab (214/216/216) soles on the German websites that are available in different heights; there's also some Ergon (IP3) branded ones made by Solestar, but they don't have any options. Both are around 30-40€.
Spec BG insoles are my option, I use the blue ones, easily available online.
I've also got some custom SuperFeet that I suspect would be really good if they were done by a good fitter. Sadly, these weren't so they don't really work at all.

Problem with mail order is until try an insole in the shoe I won't know what arch height I need.
In a Shimano MTB shoe I wanted the higher arch, in my Fizik road shoes the lower arch - guess is a different last which already has some shaping on the road shoe.
