Just about to adjust my gears and I always forget.....;-)
On the lever, which direction does the adjuster turn for smoothing out the gear shift....its the last 2 smallest cogs...
It depends what's wrong with it 🙂
For all barrel adjusters, viewed from the cable side, anti-clockwise will tighten, clockwise will slacken.
If it's hesitating to shift onto a bigger sprocket, tighten it. If it's hesitiating to go onto a smaller sprocket, or is constantly trying to jump onto a bigger one, slacken it.
On the smallest cog, moving up a gear its very noisy and does not shift up 🙂
Also when on the largest cog it does not shift down a gear. Thanks. 🙂

I usually disconnect teh cable and start again though if it is slipping
May be a worn cassette/chain - more likely the later exacerbated by the small cog size??
Turning the adjuster won't help. To fix moving up from the smallest cog you need to turn anti-clockwise, to fix not coming off the big cog you need to turn it clockwise.
Its possible your cables are all gunked up with dirt, and need flushing through or replacing. Or the mech is bent.
What spooky said - barrel adjuster won't help if you have both of those problems.
If you've not got full outers, here's a handy tip for cleaning the cables:
Shift to the biggest sprocket. Now, without turning the pedals, press the shifter as if to move to the smallest sprocket. You should now find that you have enough slack in the cable to remove the outers from the stops in the frame. You can then slide them along the inner and give them a good clean.
Remember that rapid-rise (bottom gear normal) mechs use tension in the other direction....just when you thought it sounded simple.
